Currently in my late 30s I will say even at my worst I always held the door open for people as well as said SALUD when someone sneezed amongst other forms of common courtesy. (Not religious so I won't say bless you. Istead SALUD is another way to respond which means HEALTH)
That being said there is a "across-the-parking-lot" rule but yeah it does not count here being you were already on the same sidewalk as well as no more than 10 steps behind him.
